OVERVIEW: The Case Associate works directly with the client population of a specific program implementing

the treatment plan as determined by the department's clinical staff.

RESPONSIBILITIES, un der direct supervision, include but not limited to:

  • Routine community contacts on behalf of social worker, e.g. school reports, medical reports.
  • Locate and ensure appropriate community resources for clients.
  • Evaluate actual living conditions of clients through home visits
  • May interpret agency function to client, and demonstrate this in terms of concrete help.
  • Conduct home visits in which the bio-psycho-social needs of the client is assessed, and in which help for clients obtaining needed services is offered.  Help clients to maintain medical, financial, and social resources are examples of such services.
  • Provide clinically focused case management to clients.  Identify and provide access to specific appropriate concrete services, when necessary or requested, and are in the best interest of the client.
  • Perform community outreach and establish linkages with other groups, individuals, or organizations, which are in need of our services.
  • Provide structure to the client, evaluate and assess usage of independent living skills, and provide guidance in these areas to the client.
  • May run groups, under the supervision of the social worker.
  • May participate in intake, assessment, discharge, and follow-up treatment conferences.
  • Other duties as assigned.

Job Requirements

BA Degree, with some experience in a community or social service group, or at least five
years experience as a Case Assistant in a community or social service group, etc.
