Social Worker

Summary:

Hamilton-Madison House Smith Houses NORC seeks a Social Worker to provide casework, counseling and service coordination for clients of Smith Houses NORC. This position reports to the Director of Smith Houses NORC.

 

Minimum Qualifications:

·         Masters in Social Work or related field - Counseling, Psychology or Gerontology

·         Two years of experience working with older adults

·         Bilingual English/Cantonese or Mandarin required

·         Proficient in MS Office

 

Responsibilities:

·         Provide case management, case assistance and the development and maintenance of care plans in English/Cantonese or Mandarin

·         Input accurate worker service log information into DFTA electronic system (STARS) in a timely manner

·         Conduct health indicator survey and diabetes and heart disease benchmark via STARS

·         Complete and maintain accurate clients-records and monthly and quarterly reports

·         Provide individual counseling and group work in English and/or Spanish languages for senior residents of Smith Houses. This includes but is not limited to addressing depression, anxiety, bereavement, psychosomatic illnesses, family reconciliation, dementia, maladaptive behaviors, cross-cultural barriers/stressors.

·         Assist clients in the coordination of care and services from various home healthcare agencies, benefit and entitlement agencies and legal and financial providers.

·         Coordinate services with health professionals including Primary Care Physicians and in-House Program nurse.

·         Conduct home visits and assessments

·         Assist in the planning and implementation of new educational and recreational activities for the members and residents of Smith Houses

·         Conduct outreach to community

 

Compensation:                 Commensurate with qualifications and experience

Hours:                                   Monday to Friday, 9:00AM to 5:00PM, 35 hours per week
