Director of Social Work

We currently seek a Director of Social Services to direct and supervise social service programs. In this position, you will:

•           Evaluate the social needs of each resident admitted and formulate a written plan of care;

•           Review and revise care plans;

•           Refer residents and families to appropriate community resources

We require:

•           Bachelors’ degree in social work; Master’s degree preferred.

•           Experience with Sigmacare and MDS completion required.

•           License/Certification/Registration in accordance with New York state laws and regulations required.

•           Knowledge of Federal, state, and local regulations, particularly Medicare and Medicaid regulations and long-term care operational standards required.

•           Experience in a Skilled Nursing Facility required; assisted living experience preferred.

•           Minimum of three years of experience as a social worker with the geriatric population preferred.
